Dear Park Shore Families and Staff,

In an effort to minimize illness at school, we ask that you complete a daily assessment beginning December 24th – January 2nd prior to the start of you or your child returning to school. The best school session starts with healthy staff and children and this begins at home. Please bring this completed form to school on the first day when you or your child returns back after the Holiday Break.
